Once you’ve done this and completed the expansion, complete the main game’s ‘Through Pain to Heaven’ quest.
You’ll need to wait for Reed to contact you, who’ll eventually invite you to see him before his departure from Night City.

Next up is the ‘Who Want To Live Forever’ quest which will see you waiting around for Reed to contact you again.
At this point, you’re not too far away from that secret ending.
If you took the necessary steps to achieve the secret ending, Reed will be there when you awaken, explaining what happened during the surgery.
